Dear participant

This is the first initiative to evaluate the impact of headaches in Switzerland.

This survey is part of a medical dissertation at the University of Zurich and a master thesis at the University of Basel under the guidance of the Patient Organization ‘Migraine Action’ in close collaboration with the Swiss Headache Society.

The questionnaire is validated and had been used in the Eurolight initiative to evaluate the impact of headache in Europe with 28 partners of 16 countries, all major patient associations, headache experts, scientific headache organisations, the European Commission and supported by ‘Lifting the burden’ a partnership activity of the World Health Organisation (WHO).
 

What is the purpose of the study?

Your participation in this Headache survey is for the benefit of yourself and other headache patients. The obtained information will contribute to evaluating the needs for an optimal disease management and ultimately improving headache services and quality of life. The data will be used for publications in lay and scientific literature.
 

What do you have to do?

Participants need to be over 18 years of age. If you agree to participate in this survey by filling in the questionnaire, it is important that you try to answer as many questions as possible. Filling in the questionnaire will take about 15-20 minutes, you will be automatically guided through. If you would like to save your answers, resume later or correct some answers you can always do so. When you are finished you can press ‘submit’.
 

What is the level of privacy?

The information you provide us with will be confidential. Any possible personal data will at no time appear on the database. The data will only be used for medical research and will be analyzed strictly anonymously and confidentially.
